To request an on-the-day telephone appointment, use the form below. A clinician will contact you in the morning (AM) or afternoon (PM), depending on your choice. Please keep your phone nearby and be ready to answer, as the call may come from a withheld number.

Your request will be triaged in the same way as any other appointment request. If it is more suited to another service, such as a pharmacy, community clinic, or self-care option, you will be directed to the most appropriate service for your needs.

Before completing the form, please check our Right Care, Right Place page to see if your issue could be dealt with elsewhere. Many common conditions can be safely managed through your local pharmacy or other NHS services without needing a GP appointment.

When completing the form, provide as much detail as possible about the reason for your appointment. This helps the clinician prepare before calling you.

This service is for telephone appointments only. If you require a face-to-face appointment or another type of consultation, please contact the practice in the usual way.

The form is available daily between 5:00am and 8:00am. If there are no appointments available, please try again the next working day or contact the practice as normal.

Please note: This form must only be completed for the patient named on the form. If you are requesting an appointment for someone else, or for more than one person, a separate form must be submitted for each individual.
