You are being asked to send photographs of your condition to a your GP, to help them decide the best course of treatment for you. The photographs will be stored as part of your electronic health record and will not be used for any other purpose.
Sending the photographs means that you (or the parent, carer or next of kin), understand that the photograph(s) will be added to the relevant electronic health record. The photographs will be used to help identify a diagnosis as part of a virtual clinic.
